MEDICAL SERVICES: Insurances accepted: Medicare part A/B. Private Insurances, Private Pay
Wondering how to receive our medical services? Here is some information on how to get your home health care journey started. If you have any questions, please do not hesitate to call us 970-263-0202. We are here to help you or your loved one.
1.You are under the care of a Doctor.
Having an established primary care doctor is vital to receive home health care. Doctors follow care, approve orders and work side by side with us to ensure you’re getting the best possible care.
2. The doctor certifies that you need:
- Skilled Nursing and/or Therapy Services
- Temporarily Homebound
Medicare requires homebound status.
To be homebound means you have trouble leaving your home without help because of an illness or injury, or leaving your home isn’t recommended because of your condition.
3. Documented Face-to-Face encounter note from your doctor.
SummitWest Care will work with your doctor to get you the services you need. A face-to-face encounter must occur. This means that you have seen your doctor and they have documented the reason homecare is needed.
