Having trouble with your vision? You're not alone. More than 6.5 million Americans over the age of 55 experience vision loss. Our Low Vision Program provides the tools and training for increased independence when your vision begins to fade. Our Low Vision Program offers many adaptive devices and techniques to improve your vision and to help make it easier to cope with daily life activities.
VitalStim Therapy is a safe and effective, FDA-approved treatment for patients with swallowing disorders. The goals of this program are to strengthen and re-educate the muscles in the throat and improve motor control of the swallowing mechanism. Research shows this method has a greater success in regaining normal swallow as opposed to traditional methods of exercise and diet modification alone. Our Swallowing Program Therapist is certified.
Personal Safety Alerts (PSA)
SummitWest Care provides our patients with a Personal Safety Alert System. This is helpful with patients who are not steady on their feet at all times, experiencing frequent falls and/or recovering from a surgery or injuries. Help is available 24 hours a day, 365 days a year with just the click of a button. This program allows individuals to live independently in their own homes with the resource to receive help quickly if needed. It is easy to use and affordable.Read More
In-Home Support Services (IHSS)
Our program will train and educate family members and friends to care for their loved ones. If you have Medicaid you may qualify for this program. The program is designed to provide care, training & resources at home for elderly, disabled or children with disabilities. We can train and educate family members or friends to care for the client while they work as an employee of SummitWest Care. If no family member or friend is available, we will train a caregiver for the patient and their specific needs.
Life Care Planning
We offer solutions to crisis situations. Unplanned events in life are difficult to deal with. We can help. Our Social Workers are trained to be the pivotal link between independent in-home care and our clients. Our Social Workers can mediate and offer solutions to crisis situations, as well as teach independent behavioral skills. Our Life Care Planning program empowers the elderly generation to make decisions for themselves. We evaluate individual needs, provide options to develop a life care and crisis prevention plan and provide and teach advocacy services.

We offer care where you need it most… at home. Our compassionate, quality health care team is ready to visit and help you in your home. We serve those who are ill, temporarily disabled and those who require long term care. All nurses and therapists are provided at the direction of an established physician's plan of care for those patients who require nursing, home health aide and therapy services. Our care providers are available seven days per week as needed to meet the individual’s specific health care needs.

SummitWest Care was developed as a non-profit, free-standing agency in 2002. Our community recognized the need for providing quality care in the home. SummitWest Care was designed to serve people in need of care in their home. Since our inception, SummitWest Care has served several thousand clients and families in Mesa County and we have had the support and donations from organizations such as St. Mary's Hospital, Rocky Mountain Health Plans and Hilltop. Because of our strong staff expertise and leadership and the profound belief that people deserve to heal at home, we have received several awards for quality and excellence in home health care service. Our clinicians have a long history of collaboration with area physicians to help their clients get well after premature birth, complicated orthopedic surgeries and chronic illnesses.
